FFXIV doesn't have the ability to switch dynamically between sound sources, and will always require the game to be closed and reopened. This happens even if disconnecting and reconnecting to the same source without something in between, as with a bluetooth speaker or headset being turned off and back on.

Yeah, I am sometimes fed up with having Headphones on the head, so I switch to my monitor's speakers, or vice versa.
But the game does not care about that at all and keeps the source it was started with. It's sad.

The easiest way is to set up Skype to not use your default sound output, but to go via your headset at all times. Sure, you'll get all the "ding! new message!" to your headset as well, but you won't have the problem of losing sound in the game =)
(I /think/ it also works by setting your headset as "default communication device" and tv/hdm/other speakers as the "default device", but I'm not sure.)
