I think there is a one time item reclaim you can use if you possibly used it and forgot.

If it is just plain missing, check inventory and all retainer spaces. I though i misplaced mine but eventually found it in my inventory mistaking it for a Hi-Elixer.

It may also be possible that you used it and didnt actually change features. If so try removing all equipment aside from weapon, log out, go to chaeacter selection and right click/hit square on your character to get a menu. It may allow you to edit.

If it genuinely is not in any of your inventory and you are positive you didnt use it, thry may give you another. Idk.

Also check moogle mail from devs, perhaps you never took the item from the mail.

It may also be possible that you used it and didnt actually change features.

I believe this is the case. I used it. but now the option to actually change my appearance has vanished.
