As both tank and healer I've performed exceptionally well and received zero comms for some runs. It happens. It's not an entitlement. Sure you can wonder why they didn't comm you but it's pointless.

Make sure you talk to the group a bit. Everyone commends a nice person
As a rule though, if I'm DPS, and a DPS doesn't fail during the run, I give them the commendation.
As a tank/healer, I give the commendation to the opposite.
I main paladin. My personal Commendation guidelines that I use whenever I dish out these goodies:
Did one dps use LB appropriately in every boss fight it was available? Instant commendation. Top priority (its my biggest pet peeve when dps don't LB appropriately or let it just go unused, so I really like to reward people who use it correctly).
Did the tank pull appropriately sized groups for our item levels and party composition? Instant commendation (for when I'm not tanking).
Did the healer dps when party hp was full? Instant commendation.
Did the tank pull too much without warning yet the healer is able to keep him alive? Instant commendation (for the healer).
Otherwise, no one gets commendations. If both healer and tank are good, its a matter of who I like more based on who talks.
Good point about Holy because it's definitely true! There's nothing like having 5-6 mobs that everyone is trying to take down, and I start spamming Holy watching them drop 15% HP or so with each shot of it. Sometimes it's funny, almost like "oh let me just take care of these for ya'll...." I wish there was a way to remove commendations like negative ones haha. Like the Copperbell HM I just did as WHM where the DPS was a BLM and Summoner. I swear that Summoner was casting Physik more than anything else, but not even on themselves, on the tank as well! Problem was no one was even remotely close to dying, tank would go from 100% HP to like 85% and she'd be casting Physik on him. I finally had to say we'll get through this a lot faster if you actually do your job and DPS.....
I usually dish out commendations like:
Tank generally first providing he was good and got us through it quickly. So roulette etc where you just want the tomes or something, did he get us through really fast without pulling too much? Usually would go to him.
Did a Bard take the time to actually use Ballad whether I needed it or not? Generally would give them the commendation since 99% of Bard's don't use it.
Was someone super helpful? Like saying it's my first time and someone clearly explaining everything for me? Would go to them.
Sometimes I'll give it to the other healer, but for some reason I'm kind of weird that way, almost like I just immediately automatically hate the other healer even if they are good!
I don't always give them though, and my friend says I'm a dick for it. But I'm like they are called commendations, if no one did anything that stood out, then I'm not going to give it to someone just because.

I give my comm based on their names. if I get a chuckle out of it, I give it to them. don't give two shits how you do in a dungeon.
*cough* That situation might easily have happened with me (it wasn't, though, since I haven't really played SMN for months outside of SMN quests to give more SCH exp :p ). The reason is because I'm so used to thinking "I have a book; I am the healer, I must heal people." I try to do my best to forget that urge, though, but it happens more often than not that I forget simple things like Fester because scholars don't have it, or that my eyes stray to HP bars too often. Sure, on the good side it means I use Eye for an Eye more often than the healer sometimes (it tends to annoy me when healers forget they have it ;_; or when the WHM casts Virus despite there being an actual Arcanist-based job in the group and the boss in question uses mostly magical attacks...), but... I'm a crap SMN, and I know it XD
Anyway, my point was that it's possible your summoner usually is a scholar, and maybe they only went summoner to get caster loot. Or to try summoner out. *shrugs* It happens.
(But I do understand the frustration of non-healers using gimped heal spells when it's not really necessary ^^; It's like... "Your healing fills up a quarter of a single auto-attack by the boss. Just... hit on it instead and let me keep people alive, mmk?")
As for commendations... I usually give them to people who are friendly and open. Or to that person who was new. Or that person who died two times due to bad RNG but didn't give up. Or to the person not raging at me for making a stupid mistake like hitting the wrong hotkey and missing my self-heal and thus dying (or, in case of no one raging, the one who goes 'no worries').
A few times I don't give them, especially if I'm on a maxed-out job and my commendation challenge log is coming up and I don't want to lose the exp. I don't always care about that, but sometimes I do. Other times, no one really stood out and the person I might have commended left the place a few seconds too soon.
People hand out comms to whoever they "see" as doing the best, not who is "actually" doing the best or putting in the most effort. It basically works like this in the minds of players....
If Tanking:
1) Is this a full party with another tank? If so, and they didn't take my role, they get the commendation.
2) Did the other tank leave early or there wasn't another tank? If the healer didn't force me to use all my cooldowns constantly to stay alive, they get the commendation.
3) Did the healer suck? I might give them the comm anyhow, or I won't vote at all. DPS only get it if I screwed up horribly and we only won because of them, or if I play that particular DPS class in here often and they did at least as good as me.
If Healing:
1) Did the tank keep the hate off me, and use their cooldowns to make my healing job easier? If so, they get the comm.
2) Was there another healer, and if so did they make it so I didn't have to do much of anything? If they didn't suck, they get the comm.
3) I can give the comm to a DPS? Why would I? Fine, since the tank left already and there aren't any other healers, I guess I'll give it to the DPS I had to heal the least. Though I guess I could just not vote.
If DPSing:
1) Was the other DPS good, or playing one of my favorite classes and not sucking? They get the comm then.
2) Did either the Healer or tank particularly stick out as being good, or keep me from dying when I screwed up? That one gets the comm.
3) No one was particularly good? I'll probably throw it at the DPS then since they don't get many comms. If they sucked though, I'll just give it to whoever. The only way I won't vote is if the group was particularly bad.
I think commends are just totally random.
Last night I got 3 commends in a 4man dungeon as a DRG. I only said "Hello!" at the start and didn't talk the whole run. I also made a few mistakes, nothing expectacular. I wasn't expecting commends at all so when I got them I was like What did I do right? lol Musta been my hairstyle! :O
I just want to point out that commendate is not the right word. Commend. Commending. Commended.

Be a Roe
Greet to the team in the begging. Most importantly, thanks everyone in the end.
A brd who actually sing...
Above things are so rare that you really need to reward these PPL.
