Crossbars switching on their own after activating L2/R2 Bar

I've been having an issue with crossbars (controller/PS4 user) for a few weeks now. To prepare for HW, I activated the L2/R2 bar for more skills. My setup is crossbars 1 & 2, which I cycle between with R1 and bar 3 which only shows up when L2/R2 is pressed. That bar is deactivated from the R1 cycling. Not sure if that's relevant but wanted to give as much info as possible.

The issue is that using a skill will sometimes cause the crossbar to switch from 1 to 2 or 2 to 1. Example...I start a pull on my DPS bar as SCH. Cast Bio II. Press the button to cast Miasma next but notice I'm casting Succor instead. The crossbar swapped on its own without me pressing R1. I've tried everything I can think of (different controller, disconnected my mouse, reset the system...etc) but it's still present. On other boards, a few people mentioned the same issue when I brought it up and said it was due to activating the L2/R2 bar. Is there any way to resolve this without deactivating that feature? The room is needed with the added skills from HW.

I haven't had this issue personally but it may be because you are accidentally tapping R1 while trying to hit R2. If you hit R1 and a button it will trigger a hotbar switch to the hotbar associated with that button.
So if the skill is on R2 + O and you accidentally hit R1 + O it's gonna flip to the 2nd hot bar instead of triggering the skill and you might not see it in the heat of the moment but notice it next time you go to cast or do a skill.
