Can you visit the following and tell me what it mentions for Heavensward?

Visit the “Settings” menu of the PS4, then select “PlayStation™Network/Account Management,” then “Account Information.” Scroll down and select “Services List,” and find “FANTASY XIV: A Realm Reborn.” Selecting it will show Heavensward playing rights.

Also did you redeem the 12-digit code while logged into the Sony account that you use to play XIV?
