You get a new one at 54. That said, I've only died during the story duties cause smn doesn't do single target well. :/ Overworld is easy if you fight 1 mob at a time. You can't group em up and aoe like you did in ARR.

I just now hit level 54 and continuing main story quests.

THe main story quest showers you in loot, if your lvl 54 you should have a full set of lvl 133 gear just from main story quest alone.
