Any fix for this? I use a Razer Kraken chromia usb headset if that helps. Mic and any non game sounds come through fine.

Game latches on to whatever sound device Windows has priority on upon launching and it sticks with it until you close the game and re-launch is unfortunately. Have to have your headset on and set as the playback device before loading the game.
This is both a detriment and a boon depending on your preference. Like Kaisha said, the game will use whichever audio playback device is set to default when you launch the client. To get game sounds to come through your headphones, make sure they're the default playback device when you start up the game. Personally I prefer to have all game audio come through my speakers while voice chat comes over the headset. To do this I always start the game with speakers as default, change the default to headphones, then start Ventrillo (or whatever client). Somewhat cumbersome, but I think it's more a Windows thing than anything else.
This is how its been since the beta, if you go to your desktop and change the output device you have to restart the client. For most people it doesn't matter, but for some of us, its really annoying. I have 3 outputs I use, my wireless headset, my main monitor speakers and my tv speakers.
Normally i would use my main monitor speaker when I play FFXIV and back when I use to raid i use to use my headset. But at night I like to switch it to my TV speaker cause I'm lying there on my bed watching TV before I sleep. It is annoying when the I start FFXIV the following day and forget to switch my output back to my main monitor and have to reset the client to change it.
I'm pretty sure this is a game thing, from my experience some games require a client restart while others don't need it, sadly FF XIV falls into the former.

If the Tank dies, it's the Healer's fault. If the Healer dies, it's the Tank's fault. If the DPS dies, it's their own damn fault.
