I am confused. I got ps4 early access code with 12 digits and I pre-order the physical copy from SE. So have i forgot to redeem the 18 digits code ?

I got mine from the se store and was able to enter the code already. Didn't get an email to do it either just saw this thread!

Although on the additional items for ffxiv heavensward pre order bonus items it's box next to it is blank.

I believe it is just saying that you got them, and doesnt list them out specifically. You should be good to go.

You will get those when you register the actual CE to your account. Early Access is not the same code.
There will probably be a grace period of time after retail release begins to register your CE properly to account for people buying physical copies, if the 2.0 release is any indication.

Ok, I just figured it would go active at the official launch date. I dont remember needing two code for the 1.0 CE or the 2.0 CE.
