There will be two new tomestones right off the bat. So yes, Poetics will disappear once Heavensward goes live.

For level 60 gear, yes.

That doesn't help people who buy FFXIV next month and need to hit i90 to do Chrys/SoF in order to access Heavensward, or who don't buy Heavensward for a month or whatever.
