Not to get too off-topic here OP, but I really wouldn't consider Pyramid Head an actual villain so much as a... well it depends lol.

SPOILERS(for those that haven't played Silent Hill anything... GO PLAY THEM!)
Considering Pyramid Head doesn't even show up in every Silent Hill, depending on which game it is, who you're playing as, and what's going on in the story I really wouldn't consider him an actual villain. He's more like an executioner, or maybe a vigilante trying to bring "justice" to those who "deserve it" in his eyes, also... he's more than likely following orders. It depends also on where the story is at, and which ending you're going for, for the games he's in as well... that can have a lot of proper insight on who Pyramid Head truly is and "can be".

If I was to pick an actual villain from the Silent Hill series depicted as "bat-&$^# crazy" as you said... I'd have to truly give to Dahlia Gillespie or Claudia Wolfe. Now those two women are truly fanatically bat-**** crazy!

As far as FFXIV:ARR having more crazy villains, I think one per expansion is enough healthy dose... so I'm hoping we see one in Heavensward (I have a sneaking suspicion we just might.)

And the genre being...? MMORPG? Or do you mean the series? As in the Final Fantasy series? because if you mean the series...

Final Fantasy games that had batshit crazy villains:
    5, 6, 7, 9, 10, 13-2

Final Fantasy games that had Sherlock Holmes 'super-scheming' villains:
    1, 2, 3, 4, 8, 11, 12, 13

I beg to differ on these two right here....

Eald'narche(FFXI) & Vayne Solidor(FFXII) were pretty high on the craziness of it all! Especially there right towards their ends!

Its funny because honestly I considered the Zilart Brothers at first to be crazy however, they successful founded and ran the major hub of Vana'diel, not to mention it was them who honestly unified the other nations, albeit it was for their own gains, but that alone gives more to the scheming arch-type of villainy than that of the crazy one. Vayne from XII was very...excessive and thorough when it came to climbing ranks and ensuring his power, which to me also gives a scheming super-villain arch-type. He was in control and a had a clear plan; crazy villains tend to not really have a plan besides "I'mma kill you".

I agree that both those characters were 'crazy' at the end but that doesn't really constitute them being crazy villains, it just means that they became desperate (most villains in entertainment medias toward the end tend to exhibit crazy behaviors but that doesn't make them a crazy villain per say)

So you are taking about RPGs in general. Well the genre Role Playing Game is pretty vague... I mean technically role playing games are identified by any game wherein you are placing into the shoes of another person (to play a role, or role play). So we can use the 3874 PS2 games as a loose point, we know that there are sports titles to rule out (soccer, golf, baseball, basketball, football, tennis, hockey, boxing, wrestling, racing), PS2 had new games being released for 10 years from its launch which means that 100 sports titles were launched, but they have competitors so give or take 300 'sports' genre titles launched in the 10 year cycle. That means roughly 3574 games can be considered RPGs (shooters are a RPG as you fill the role of a person, which is why I even listed villains from shooters in my OP). I assume you mean "over half of the titles have crazy villains" when you say its been played to death and back again so that means you are claiming that with just the PS2 games alone that at least 1787 PS2 games contain crazy villains, but since its been played to death and back I assume you mean 75% roughly of the games contain these types of villains so roughly 2680 of the 3574 RPG type games contain crazy villains? I kind of doubt that.

Ok I'll give you that on Vayne, but... Eald'narche still strikes me as very crazy from the get-go... someone who "hears the gods" speak to him, there for he wants to "cleanse" the world and bring "paradise" about? Just because you can plot, plan and scheme... doesn't make those plans sane.

I'd just like to point out that FFXIV did had one villain that sorta fits the crazy ass villain. But honestly he was a 1.0 villain.

    Nael van Darnus.

Legatus who decides to cleanse the lands by dropping Meteor on Eorzea. A clear fanatic. Although admittedly, he only began getting crazy once Bahamut began to influence his thoughts. But Sephiroth as well started as a normal person. Until he found out about his origins and JENOVA.

There are still a few prospects for a "puppet-master" type of super villain in the FFXIV storyline.

    SPOILERS

First is whoever is behind the overthrow of Ul'dah, at first I thought it might of been Taledji or Lolorito, but events in 2.55 showed that it clearly wasn't Teledji and most likely isn't Lolorito either. All we know about their plotting so far is that they desire the Omega weapon buried beneath the Cartenaeu Flats, and that the Sultana dissolving the monetarists hold on Ul'dah would of had a negative influence on their plans.

Another is Iceheart, she is more of an unknown still though. Is she actually another champion of Hydalaen fighting for a just cause, or has she been corrupted by Draconian influence?

Of course either of these plots could just as easily lead to both factions being manipulated by the ascians, but I'm hoping that at least one of them isn't.
