If they removed the limit on subscription time, then if they chose to raise or lower the subscription cost, it would create a huge headache for either refunding players excess money spent, or trying to get more money for already purchased game time.

That's a very simple solution in simply not offering refunds or price adjustments to previously purchased time cards... I don't know many time card sales that would ever even consider doing that, even if their subscription pricing changed.
and if, in 3 months, FFXIV theoretically went Free to play, would you be okay with "no refunds" on that extra time you just added?
If that's how it would work, shouldn't they then refund anyone that bought the physical game itself too? Like what if I bought the digital download the day before it went free? What if I bought FFXIV on Steam for full price when then next day they had a sale? Businesses don't operate that way. If you purchase something like a time card / gift card you aren't eligible for refunds. Pretty sure it even says it on the boxes.
