Controller stops working on Windows PC version (anyone else?)

With common frequency, my gamepad stops responding.

With my game set to gamepad mode, but also having a keyboard present to chat. Usually soon after typing anything on the keyboard, the gamepad stops working entirely. Keyboard use seems to be the trigger. Occurs randomly.

If I was moving when it cuts out, my character will continue moving in that direction uncontrollably until I re-establish control by disconnecting and re-connecting the gamepad. When this happens during battle content, I die and have to apologise to affected players I was helping.

Changing gamepad doesn't help (tested with both Xbox 360 and Xbox One gamepads). It only happens with FF XIV.

Reminds me of another issue that only happens in this game, where the video just randomly cuts out (infrequent but still bad) and I have a black screen. Another user with the same graphics card confirmed this also happens to them. (GeForce GTX 460)
