I've been saying this for a year and odd now
Im on PS3, started with the standard HDD and the slow slow loadtimes and waiting on NPCs/assets to render in open world. Then I upgraded to a pure SSD (not a hybrid) and wow the difference is staggering, seriously load times are seconds between zones/dungeons, FATES and NPCs/PC's load near instantly in open world.
Only frame rate grippling event still (so far that I'v played) is the Odin FATE if theres loads of players lol.
Probably means they were teleporting when the DF prompt popped up and time out due to slow loading into the zone.
EDIT: Nvm someone beat me to it xD
That happened to me once, or twice, or loads of times. On pc though.
So I had to upgrade to 16 gb ram memory and now im never raging again

This happens to me on my crap laptop.
Can't do much about it till I have enough for upgrades...I get locked out this way all the time.
My advice if you can't afford an SSD? Just don't teleport....simple.

You get what you pay for?
I agree that it's worth the $70 or so to put an SSD in your PS3 to speed up loading and teleports, though if you have lots of stuff on your PS3 it may not fit on a small drive.
It's also worth it to get a PS4 if you can - then you can actually see all those people you're "hearing" in Revenant's Toll.
