Your use of weaving off-GCD skills with GCD skills is poor.
Especially Swiftcast. See page 4.

You're wasting 1 off-gcd.

I wouldn't advise using PoM & DS together unless you're a particularly undergeared character facing specific mechanics like; T4 Dreadnaught, T9 Favour.

I wouldn't use swiftcast in this rotation anyways, as you just used PoM the MedII will be more in synch with the GCD meaning even without swiftcast you could fit in 8 GCDs in DS.
Or am I missing something here?

Btw, I would not use DS and POM together too.
If you still wanted to, you could take the rotation and rearrange like this:
[SwiftCast] > [DS] > [Med II] > [PoM] > [CureSpam]
or don't waste that swiftcast:
[PoM] > [DS] > [Med II] > [CureSpam]
