What we know so far, Materia will be gained by the destruction of gear that has filled up a new stat called "attachment".
The stats on the materia created will be a distillation of the stats of the piece of gear it was created from. The amount of carry over is still not fully known. The materia can then be fused with a new piece of gear of the players choosing via crafting and there will be 0 chance of the synth failing.
If you decide that you are not getting the bonuses you want you can have your fused materia removed, they will be destroyed in the process though. There also hinting at a pretty complex system of placement bonuses to be in place so its likely a lot of trial and effort will go on early on.
Materia its sellf will have a chance to hq on creation. we dont know as of yet what that will mean only that a hq materia will be significantly more potent then your run of the mill version. there also talking about allowing us to "Double slot" in materia, how ever failure will be possible and it sounds like the cost of failing while doing this will be high.
In short for the tldr crowd. No you wont be able to slot a materia into your undies and then be able to summon ifrit or any thing wacky. you will how ever be able to make them super strong and defensive.
