I don't know... this is the norm on JP servers, probably because it's easier than typing a a name in romaji. So it doesn't bother me. If someone has a long or weirdly spelled name, it's annoying.
i can understand with long names or difficult names, but I've been called "bard" rather than my character's name, which isn't hard to spell or pronounce, often enough and had enough bad experiences related to situations involving it that it does get on my nerves.
I think it is just people looking at the job rather than the players name, I wouldn't put much thought into this.
Same here. Sounds like the person is talking about you, but not with you, enforcing the fact that he/she doesn't have a good opinion of you. I always take the time to look at party mates' names, and if you don't care enough to call me by my name, I won't care to give my commendation to someone else...
For JP servers, I think it may be an extension of how people interact in real life. We often call professional people by their job when we don't have any personal connection with them... so it's not considered offensive.
I've never been in a DF party with English speakers who used party chat, so not sure there.
I have been known to do it. I keep my party name plates off (the ones above heads) and in instances don't like to glance at the party list so I don't have to worry about missing a telegraph

However, I remember back in 2.1 when Pharos Sirius was people's nightmare and we just got it, someone said right away "Does our healer know at least what to do?" and I got offended first because it was a 3rd person question and second because it is not that hard to read what Siren's debuff does on first try <__<".
In party names are off for me. In a pug, I often don't know the names. It's also obnouxiously annyoing that cast bars ruin the party hud, you can't see the name for a second or two.
It doesn't bother me at all when someone refers to me with <job name>. I always try to check someone's name but sometimes you need to be really fast and job name is the better option. Not to mention some people might have, for me, weird names.

"Bard, could you sing?" Is a bit more understandable than "I hope the bard sings."
If I wanted to be treated like an impersonal label I'd go to the DMV
