I can always work to be better on my rotation for sure ^^ thx for the answers everyone! It's not a bug, your question has basically been ansered by Synestr in the 3rd post.
The amount of active debuffs on a mob is capped at 60, which can be reached pretty quickly in ST or with A/S ranks, especially when healers help dps'ing etc...
Shouldn't be an issue at all though if you do not let your DoT's fall off, since if you reapply your own DoT it overwrites the prior one and doesn't count as a new one.
Maybe it's an issue with your own rotation and your DoT's fall off from time to time? No offense, just an idea... ^^
To sum up what others have said and to clarify some of the resulting questions:
All enemies have a display cap of 30 effects (whether buff or debuff) that will display at one time. This is not something that should be happening in a Titan run unless maybe your group has everyone trying to hit all their debuffs at the same time or something. If I had to hazard a guess as to what's happening in your Titan example, I would say it was either player error or server error, because a dot should land on Titan. Your Syrcus Tower example most likely is hitting the 30 effect limit. More important than that, though, is whether it's hitting the 60 effect limit that prevents any new effects from applying. The only way to see if you're in slots 31-60 is to see it in the log or the floating text; filtering to only show your own effects does not work. In ST, you might be hitting the 60 limit, but you'd have to confirm that yourself.
On a related note, this is part of why I don't like healers throwing Aero out during hunts. Toss something that doesn't use up a debuff slot, please. . .

On a related note, I believe there is a settings option that allows you to turn off all debuffs from other players. With this checked, all you see on the boss will be your own buffs. Helps tremendously in LoA/ST and FATEs like Odin and Behemoth.
Note that just because you can't see other players DOTs doesn't mean they aren't there. If you have this setting enabled and still can't see your DOTs then the boss is probably sitting on the 60 cap.

I test that in syrcys tower and you still can not see your own dots if there is more than 30 debuffs already. That option do not help anything imo. I think it help more if you can see other players debuffs so you know better once there is good time use your dots.On a related note, I believe there is a settings option that allows you to turn off all debuffs from other players. See im glad you said this! I was just thinking wait why is this happening in my titan runs, if there are only 8 players there actually is no way we have 60 dots running at one time, so what gives in these instances. And it very could be player error on my part, but its happened a couple of times that i had to ask the community to check this out.To sum up what others have said and to clarify some of the resulting questions:

As for Hunts/CT/ST mobs, you can get all your DoTs fully visible by changing your opening slightly.See im glad you said this! Instead of using Bio2 -> Miasma -> Bio, do Bio2 -> Bio -> Swiftcast => Miasma. DoTs have different application speeds post animation, Bio being the slowest while Miasma is the fastest application hence why doing this rotation will guarantee you can get all your DoTs up in time, and they will all be fully visible. After that, just simply refresh your DoTs before the timers fall off so you will never lose track of them or get gimped by the 60 DoT mob limit.

guy guys guys! I think we have a misunderstanding cause we keep getting a little off base. Just gonna nudge it back in the right direction
At certain times in fights, usually on bosses be it 8 man or 24 man raids. I cant apply dots.
Now we know why this is so for 24 man raids cause bosses easily sometimes reach the 60 cap on dots.
However this is not at all true for titan or any primals at all. You can have 4 dps all summoner and it still wouldn't come close to hitting the 60 dots cap.
Im saying that my dots sometimes can not be applied at all. They have animation, but do not pop on screen and no damage ticks are shown. I am now very certain its a bug, for at least the primal/other boss fights.

Unless you're applying these DoTs during invulnerable moments in titan, perhaps it is a bug. I recently did titan about 3 days or so ago and I didn't run into this issue, so I really can't say more then that.guy guys guys! Titan is usually immune against every attack after heart phase if you tried dot at that moment. I have never seen that kind of bug myself.

Titan, Garuda and Ifrit all have a brief invul phase around the use of their ultimate ability which casuses no DoTs to land and all damage to be negated. You'll see a spam of 'Invulnerable' messages next to your target where it would normally show the DoT being applied. Yes it's annoying.
With regards to the DoT cap and display issues, this one has been bugging me for ages. If you apply a DoT anywhere within the 31-60 slots it will not show for you despite being applied. Even if you adjust your UI settings to show your DoTs with priority and even if you disable all others debuff effects you STILL won't be able to see your DoTs. For SMNs this is particularly frustrating as we are unable to see when we need to begin reapplying them... Other jobs apply their DoTs as part of routine rotations but SMNs need to do it based on the timers. I'm hoping SE can do something about this at some point but it looks like a UI issue with how it accesses the array that stores all the debuffs applied to a target; potential PS3 memory issues... sigh.
