No hate here. Unfortunately this is a pipe dream as this style of game is not set up for a system like this. FFXI the "difficulty" was the systems you played with, not mob behavior or anything to do with the mob. Autoattack whack for 2 min, coordinate one skill with your team, attack. 1.0 was closer to a system like this (1.23 was actually pretty good happy medium) but all that is now gone. I hear some people are running 1.0 servers and that might be your best hope for a "hardcore" server.

Diablo was built with hardcore servers and systems in mind so everything already was there to support it. If you say that FFXIV is built this way then do the extreme coil mode and if you get echo buff, turn it off. Sorry if this sounds like hate, its not. I just dont understand the mind set of "I love this game, if you just changed everything about it to be like X." This kind of idea was floated around since 1.0, its nothing new. I'm terribly sorry but this is not the style of game you are looking for.

And how would you react if your healer decided to be nasty and just let your level 50 chr die in a raid then ditch? These ideas may sound nice in theory but in a game like this, highly impractical.
