I'd suggest what was said above, Legend of Zelda spirit Tracks/Phantom Hourglass is really fun, and fully used with stylus controls, so no second hand is needed at all. Any turn based FF game could theoretically be played one handed. There's a slew of FF titles on smartphone/tablets as well.

FF1-6, (ff4: After years as well), FF Tactics: War of the Lion, and Final Fantasy Dimensions are all on iOS, not sure about android, though I'd assume they would be

last year, I had a brain stuff damage that paralyzed my left side, all good now except for my left hand, is SO DANG LEFT lol, for normal stuff is ok, driving, handling objects etc, but my skill to play more dexterity stuff like ...uhhh, mario bros 3, nope, still playing like a toddler

So to keep playing FFXIV I started leveling monk, using a ps3 controller on my pc, so, mouse/ps3pad combo so I can use the UI more easily with the mouse and asing the actions to the right side of the pad and reconfiguring the button mapping made me easier to play monk, not to be the top dps tho, but the stance otation made me easier to not screw combos and so.

The less used actions I asigned them to the other side of the pad, and nowdays that my hand is more responsive I still do that to the other jobs I have been leveling. I think BLM could work too with one hand.

Well, if you don't mind your RPG's being in the Sci-Fi genre, there's Eve Online. You could play the entirety of that game with just the mouse (keyboard is mainly used for hotkeys and chatting).

"I've been playing MMO's a long time and if there's one thing I've learned, it's that lions do not concern themselves with the opinions of sheep. Just take that little voice in your head that tells you to be tactful and understanding and shoot it...shoot it in the goddamn face."

    One-handed?

I suggest Terra Battle for iOS/Android. Developed by Mistwalker with soundtrack by Uematsu. Beware this is not a game you can marathon unless you shell out cashmoney - there's a stamina system in place that regenerates at set intervals and at certain parts of the story.

One MMORPG that comes to mind is Atlantica Online, it a turn based MMO and can be played with the mouse alone....true is a F2P that relies heavily on cash shop, but you don't need it unless you want to pvp (there's no PK or forced pvp in the open world, so PVE can be played safely without the cash shop).

Outside the RPG spectrum, if he likes turn strategy games, he can check out the Civilization franchise, great games, only the mouse needed.

For MMOs i can recommend Star Trek Online. Their space combat can be done with a mouse. It's tricky but possible.

From personally experience I actually somewhat injured my hand in a car accident once (thankfully it was only a pinched nerve or strain or something. Didn't show up on the initial x-rays and went away after a week or so) and managed to play through most of Suikoden V (again, "most" because i DID recover rather quickly) with one hand. With practice most PS1-3 games can be played one handed if you hand is big enough. Basically any Shin Megami Tensei game will work as well. They've got great stories and gameplay and require more thinking than dexterity.
If you have a PC, the new-ish Shadowrun Returns games only require one hand as well. Actually quite a few new and interesting RPGs from 2014 only require one hand: Shadowrun Returns, Divinity: Original Sin, and Wasteland 2. They're not overtly graphic intensive either so you can run them on pretty much anything.

Just like everyone else said - pretty much any turn-based will work. There are some older/remastered Final Fantasy games on steam and some (I believe I saw origins (FF1 + 2) and FF6 in the PS3 store And I think FF7-9? I forgot.
Highly recommend Suikoden/Suikoden II for turn based RPGs in the PS3 store if they're a fan of older RPGs

As far as MMOs, Ragnarok Online (F2P) can be pretty clicky (click to move, click to attack (you can use /noctrl as a setting to keep the character swinging on the target automatically), click to pick things up, click to talk to NPCs, etc) you can use the F-keys to use skills but you can also just click them. A bit on the older side, but I used to mouse it when I'm being lazy. I'd suggest melee DPS classes, though (assassin/knight/monk/taekwon/etc), since the caster classes (wiz/sage/priest (naming non-rebirth jobs)) when soloing is a lot of hit and running. And archers - before hunter trap skills - are pretty much hit and run (circling-bard syndrome) as a norm when soloing.

Don't Starve, on Steam/PC isn't an RPG (more like a humorous Tim Burton-esk toony survival game) but it's pretty amusing and can be done completely via mouse click. It's easy to use and easy to entertain oneself/pass the time with (except if you die. THen it's not so amusing. :| )

If don't mind free to play and super extreme Chinese grind games, there is Myth War 2. The combat is turn based, movement is done via mouse (like the Diablo series), and using items in combat can be done via mouse clicks or hotkeys.

I used to play the original Myth War quite a bit until they shut down the servers to use them for Myth War 2.
