Frequency:
Always. Every time I log in, it doesn't matter what time. It has occurred as frequently as once every 30-45 minutes after log in to between an hour or two. Usually up to 5-10 times within a 6-8 hour period or more.
A majority of the time it's error 90002, which allows me to immediately log back in unless the server is full. Most of the time the server is full. Much less frequently it's error 90000, followed by error 3102.
I feel it's important to mention the majority of this problem began with the implementation of patch 2.4. I had errors before, but never at this severity.
It tends to happen noticeably more often between 7pm EST and 1am EST.

That would be an EXTREMELY long list and outside the span of my memory. But just as often I am not in combat at all when it occurs. Such as when I'm crafting, conversing or idling.
In-Game Time:
This has happened probably around 75-100 times. I honestly cannot say I was looking at the in-game clock, but it's been at all hours.
Steps:
Most of the time I am healing as either Scholar or White Mage, but the same error reproduces no matter what job I am currently set to. Doing anything from The Second Coil of Bahamut to other miscellaneous duties. It has occured also while I've done Treasure Maps, Ixal Beast Tribe quests, and Battlecraft Leves (of all levels) or while participating in FATEs and The Hunt in multiple zones. So far as I have seen it has nothing to do with what I'm doing at the time, as it happens regardless of what I'm doing in far too many different situations to be pinpointed to one particular thing.
It has also happened multiple times while crafting, losing me considerable amounts of gil in lost crafting materials.

I've taken steps such as application forwarding, port forwarding, trying alternate DNS servers, updating (and downgrading) audio, GPU and internet drivers, trying different versions of DirectX, un-installing and re-installing the FFXIV client and testing the connection on an entirely different system. Nothing has alleviated the problem and it's now becoming a serious issue insofar as going through coil with my progression group. Or...well pretty much doing anything. I've exhausted every option I could think of.
It's coming close to rendering the game unplayable at times. I know you guys have a lot to handle, but the sooner this is addressed the sooner you'll have a very happy customer.
If there's any more information I can provide, please advise me.
