I suspect this will apply to the ceremony settings but not to the actual outfit itself, which will probably be according to that particular person's band.
My guess at the answer to the OP's questions, based on what I see at the site...
1. You'll probably get the outfits according to the gender of your character since they're gender-locked. The two outfits are the two variations, both of which are dyeable. (I'm guessing you may be able to trade if you gender-swap later, as you can for some other gender-locked outfits?)
2. Per above, the ceremony settings you can choose are based on the lower of the two plans. (But I do think the outfits will probably be according to the band you're using.)
3. Platinum includes everything that's in the earlier plans. This comparison chart makes that more clear.
