I'm aware a driver or the video card itself might by causing the issue, but every other game I play that recognizes multiple monitors does so correctly. I have rolled back and installed alternate drivers to fix the issue to no avail. Again, it is isolated to FFXIV which makes no sense.
I agree with the blackscreen... could be a lot of things. I have worked on process of elimination via testing, as much as I loathed to do so, to no avail, and again only FFXIV is giving me any issues at all.
I disagree with comparing game behavior. Known good is a viable method of testing and is good for testing general issues off hand, though I also use benchmarks like 3dmark which are even better (and a favorite of overclockers like myself). You're also being overly dramatic about video cards. I'm not a dunce. I have a huge amount of experience and only posted because I was utterly stumped and figured someone might have had a similar issue. You'd be surprised with what random forum heroes come up with sometimes (and that's not you right now). I appreciate the help but you're coming across as arrogant with that statement.
Call BS to whatever you want, but do try to infuse logic into your random rants of hatred. DAI does have a lot of bugs and a vast majority of them are related to gameplay and thus not related to our topic. I have seen the alt tab bug which is little more than a nuisance and an occasional closing of the game but not a crash. This happened to me twice in a little over 100 hours of gameplay. Nvidia's new drivers have actually increased performance for me by a little bit, but I'm aware of the issues with performance on nvidia cards and was aware of it before the game's release (I was an avid member of their community). The only thing I don't have maxed is antialising which I toned down a little and while I'm not at the point where I think I should be (and many agree), that's a different discussion related to that game.
