It will then ask for your prefered method of contact, proceed with your choice fill out the form then use the method of contact that they supply the information to. Outside of that I dont think anything can be done.

I'm mad at Sony because I don't understand how they let me upgrade a game that according to their own purchase history for my incorrect account I never bought to begin with. Because the PS4 upgrade as far as they can tell isn't a "purchase". I have no receipt or transaction number from the PSN store were I put in the voucher code. Like do they let people buy DLC for games they don't have? Then I'm equally mad that the support person I spoke on chat with swore up and down that deleting the psn user from my PS4 would solve this problem when it most difficulty did not.
