I believe Aion was like that, a yearly event would happen for a ticket for a gender switch.

I think now they just let you do it anytime, I haven't played Aion in a long time so I don't know whats going on but last time I believe there was a npc in the bar and you could change anytime you wanted.

I could be wrong though about the anytime thing like I said haven't played in a long time.

Actually, Aion was the game I was referring to earlier in the thread. You can now purchase appearance (Plastic Surgery), Gender Swap, and Name Change tickets on the website cash shot. These are separate purchases, though, and to buy enough of the "coins" needed for all this...is roughly $50. Break down is about 20 for the plastic surgery, 10 for the gender change, and 20 for the name change.

Guildwars 2 regularly gives away things that are in their cash shop- mostly as birthday presents or special event giveaways.

I do believe they should give out a fantasia in certain circumstances.

a) new race added to the game - this is 2 fold... one is give everyone a free shot at the new race and two... to change back you actually need to buy a fantasia
b) maybe once every 2 years during a certain event/be it in game or out of game/ give all a fantasia to allow people to spice things up
c) come up with some silly rep grind or something that would at the end fetch you a fantasia - repeatable after a 30 day cooldown

Just throwing stuff out there. Don't care at all. I burned mine up when I wanted to do minor adjustemnts to my character along with a hailstyle change.
