I am a tad confused. My phone just got notice of an OS update, do I need to remove the token, update and then re-add the token? And, does this have to be done every time an incremental OS update comes out?

I think so, at least that's why I had to do with my Samsung. If you don't remove the one-time from your SE account and update your OS, the app will reset and you have to either use the emergency removal code (if you saved it) or call SE Customer Support to have your account unlocked.
Make sure you remove the token from your actual SE account. I know that's kind of self-explanatory but I've seen players get messed up over it, hehe.
