I have seen a fair amount of Ninjas falling (pun not intended) in love with the floor recently, but to be fair, I've seen a lot of stupidity all around lately.
-WHMs that don't understand they need to bring up protect on people who died from the previous fights
-Tanks that don't understand what it means to hold aggro instead of humping the bosses leg for ten minutes in hoping that it starts swatting at them first
-lolBRD with "What's foe's requiem? I'm supposed to use songs?"
-Scholars that don't bother to resummon Eos after she gets ripped apart by adds
-OT that don't understand they need to GET adds
-Tanks in general
-Why do I keep running ST
-Oh god please send help

The OT thing in ST is getting to be a pain it's like all three Tanks decide they are MT so when tons of adds appear and start chasing DPS and Healers they are all busy fighting over boss aggro. Last night had two clones attacking me and I'm "Where the hell are the OT's?" in chat while trying to stay alive the MT informs me that they are trying to take aggro from him even though he established on the first boss he would MT so the OT's finally figure out they should be doing something about the adds and one of them is all "Oh shut up nobody dies from clones anyway" Apparently it was rude of me to expect somebody to do what they are supposed to be doing XD.

I think it is a combination of level 50 NINs being undergeared and being unfamiliar with their rotations to the point that they're focusing on them so much that they get killed...

Id rather they mess up a rotation and get out an aoe instead of dying imo. Your dps = 0 when dead.
