As far as I recall, You get one from the first encounter with an Ascian, around level 15 in the main quest. You then get one from each primal fight, Ifrit, Titan, and Garuda. The sylphs give you one when you investigate Ramuh, since he is not summoned at that time.

Wrong. You get your first one at level 5, at the Sultantree/Lifemend Stump/Seasong Grotto fight (depending on your starting city) That's the Water crystal. Ifrit, Titan and Garuda each give you their appropriate crystals, yes. The sylphs give you the lightning and you get the ice crystal for beating Stone Vigil. So when you beat Garuda you'll have all six elements (Maybe I'll manage to do it this weekend...). At level 14, at the Unholy Heir/Guardian Tree/Swiftperch fights, you get to see a Dark Crystal after you beat the Ascian, but you don't get to keep it--it shatters when you bend down to take it. It's also the second time you see him--you see him before at level 9, at the Sil'dih Excavation Site/Spirithold/Woad Whisper Canyon fight where he conjures a golem.
