Items Missing from Inventory

Date: 10/24/2014 Upon Sign-In early morning
Frequency: Sometimes. It has happened at least 3 different times with other items, but this item being more important and expensive, was easier to notice and had to mention.

Class/Level: Botanist Lvl 50
Coord: Unknown
Party or Solo: Solo
Steps: Logged into the game, changed to Botanist using my Gear set ad saw a notice saying that all gear could not be found. Saw that my Botanist Doublet HQ was missing. Previously in the past I would see a message that I didn't understand tell me that an item could not be found but a another one of a different color was used. This has confused me as previously I had the same gear, or it appeared to be the same gear. I don't know if the person who crafter the item was blacklisted, changed servers or something and any item that had their name on it was also moved or purged from the game.

Are you certain its not misplaced in...

Your retainer's inventory, your retainer's equipped gear, your inventory, perhaps you converted it to materia by mistake?
There is no way for items to simply vanish and PC Specs have nothing to do with it, also stop using Internet Explorer.
