Guess I should've specified hand was which. The one getting the surgery is right, and I'm pretty much planning to stick to the left half of my controller. While keyboard is a possibility, I don't have a place for a mouse, even if I could adjust to using it.

Yes you can reassign the controller binds on PS3/4. You will of course want to make the adjustments and get used to it before the surgery. you could level lower level combat jobs would have to set the controller down to rotate the camera or use target lock to turn it. Looking at the lodestone your archer would be a good job to take to 30ish, can do the commands all on the left 4, using L1 and L2 for macro bars indstead of L2 and R2. You'll lose autorun but not a huge loss. Set L3 for target lock so you can use it to turn the camera fast.

When I'm lazy I usually only play with one hand using only the keyboard, it's possible to play most primals, ST and regular dungeons. I can't do Titan as I don't have strafe bound so any direction other than forward takes a while.

I frequently play a second game with my left hand when doing something easy or monotonous in FFXIV with my right hand (gathering, for instance, or crafting a ton of items in one go, or beating up weak mobs for my daily Hunt registry). I've found that it's actually pretty easy to access all the controls on the controller with one hand if I rest the controller on a surface (I use the arm of my recliner) and place my hand on top of it. I manipulate the movement and camera controls with my palm, and press the control pad and buttons with my fingers like they were a keypad. It gets very easy with practice, though I wouldn't recommend trying anything too intense.

The only times things get awkward is when executing hotbar commands that require stretching my hand all the way across the controller - for instance, L2 + square button, or R2 + control pad up. I try to keep frequently used actions away from those hotbar configurations.
