There wasn't any crash issue (in this game) in 344.11 or .16, just the fog corruption. Those are the drivers from October... as in the latest ones. So, it may have been a driver issue that just got fixed. As noted earlier, he is using older drivers and the the other user that is also not having the issue (Bob) was also using a release from October as well.
It may not necessarily be the video drivers per se either--it could be audio, mouse, some odd legacy IR device or an HID drive, or even a background service that is causing the access violation that is crashing the game.
In case anyone was interested..
On the latest drivers, still same issue. I'm going to try to do some RAM configuration, and see if that works, if not I have one other solution..wish me luck
So... i just wanted to let everyone know after hours of investigation, I think I finally figured it out.
I don't think I've ever mentioned I'm using Windows 8.1 Embedded. Which is more then likely what is causing my issues. I've changed the compatibility mode of ffxivboot.exe to Windows 8, and I haven't crashed in hours.. will post back if it's not fixed. Either way .. I don't have any other issues with anything else, other then FFXIV. So in conclusion I believe the way FFXIV is coded, it causes this crash.
Edit: nvm crashed again after hours.

open
Control Panel\System and Security\Action Center\Problem Reporting Settings
and set it to 'Ask me before checking for solutions'
this will add additional info to the crash window.
