If this was only in low lvl content, I could understand but this has been happening mainly in high lvl content.

What I have been seeing lately.
- OT's staying in tank stance during bosses, even when there are no adds. There is really no reason for this, it reduces your dps and makes it so you potentially end up fighting for aggro control.
- Healers using HoT spells (regen, med 2) during a pull. Healing generates aggro and a HoT means you have established a constant stream of aggro towards you, multiply that by a full party that you just cast med 2 on and you have just painted a huge target on yourself.
- DPS going all out with their attacks and offensive cooldowns during a pull and not using their aggro reduction abilities. Damage generates aggro and big spike damage can generate a decent chunk of it which can easily mess up a pull if the tank is still running to the boss to pull or has not even got in their initial aggro generation.

Has anyone else been seeing this regularly as well?
Since I've began playing in 2.0, it's been the current usage.

I tend not to bother, as I can change myself, but I have no control over others, except letting them the aggro and hopefully the healer let them die so they know. Same with the Healer. I will use my provoke to get aggro back, if someone still insist of getting the aggro back, be my guest.

It take a few shot ahead from the Tank before you begin blasting stuff or healing, it's not supposed to be this hard to understand the concept.. yet..

It's sad, but that's what you end up playing with more than just 24 people from your own circle of friends.
