I know this game requires a pretty decent machine to run smoothly, but has there been changes since the last few patches to optimize the engine for even better performance since the launch of the game?

Nope not yet....
i really do hope that they have a team working on the engine... the 12 have mercy on my soul if they are not.
no point in having the game look like a BMW 6series to have it running like a 1999 Kia Sephia
Your mileage may vary on this front. If you haven't played at all since launch it runs a lot more smoothly for sure. If you've played in the last couple months it's iffy. I personally have seen a performance increase outside of cities (and those are fine after the initial rush where the game has to load everything). Of course this might just be graphics card updates instead of the game itself, though.
from beta to now seems to me like every patch its got worse.
1 year old alien ware here.
Something else is wrong with your computer then.
Patch 1.15 iirc was a huge boon to just opening your menu and selecting items. During CE it literally would take .5-2 seconds just to scroll from one item to the other. There's no way your computer is running it worse now than it was before and updates be the cause of it.
performance has greatly increased since beta, but it could be better still. Running on a Gateway FX series laptop (almost 3 years old) and while it may seem a big sluggish at times, it is nothing compared to the crap we went through during beta and the following few months.
Healer strike is ridiculously foolish and accomplishes nothing
