It is always the same, at all times, every single time you try. All you can do is try to get some kind of credit in fate after fate until you get all 12 or quit.

I'd like if anyone can confirm that you can get an Atma on a FAILURE.
I'm not talking about getting copper or silver, but actually failing to complete the FATE even if you would otherwise get credit for it. I've seen conflicting reports about this but I've never seen someone actually verify an Atma on a Failure(Too often they confuse failed with just getting copper).
I'm specially interested now because when doing fates alone there are some fates that I simply do not have the time to complete and It'd be nice I could just tag them and let the timer run out.
I've farmed 120 atmas and all I can say is that all of mine were obtained from golded, successful FATEs. Most of the FATEs I did fell in that category, so my experience isn't exactly indicative of anything, but over all the time farming there were still quite a few that didn't fall in that category. I just never happened to get an atma from any of them.
What I am interested in knowing is if you have different odds (presumably lower chances) of getting an atma if you don't win or if you get a lower grade.
Yes, exactly the point of this thread. I'm in the same boat as you. I'm trying to get another Atma weapon and I'm finding that there are sometimes 1 or 2 people or even no others at all to help with these F.A.T.E's. Sure, I can solo one here or there, but the problem is that while i'm solo'ing it, there are other F.A.T.E's that are timing out and I don't get a chance to participate at all.
This is definitely something that I'm trying to find out also. Which is why I've been asking for those with experience to share as much information as possible.
For several hours today I have been farming for an Atma in Eastern Thanalan. I'm basically tossing a single shield lob at a mob and then running away. Sometimes I get Silver and a Failure and sometimes I get a Bronze with a Success. But so far zero Atma's. I'm still trying to encourage others to post a screen shot or video of an Atma being obtained when the situation was not Gold and Successful.
So for the last two weeks (off and on) I have been trying to get an Atma in Eastern Thanalan with no luck. During Mon-Thurs I have committed 1-2 hours a day and on Fri, Sat, Sun 3-4 hours a day. I have been purposely getting Bronze and Silver contribution on all fates in the zone and not had any luck. I know some people here have said that they have gotten an Atma with only Silver contribution, but I'm starting to think it's really just a myth. I have still not seen any screen shots or any actual proof of anyone getting an Atma on Bronze, Silver or even Failed Fates.
Is it possible that there are even lower rates based on player contribution and on success/failed fates? I have two Atma weapons and each of those weapons I was able to get within the span of 2-3 weeks. I am stuck on this one zone and it's almost been the same amount of time.

When I finally decided to buckle down and grind out my Atma, I did something similar to this. Instead of X number of FATEs, I relegated a select amount of time. I gave myself three hours each day. After the five I had already earned months ago before giving up on the whole process entirely, I actually managed to get the other seven atma stones in half a week! It's all about pacing.
Can tell you for a fact that I've gotten an Atma from a failed bronze medal FATE. Hope that clears everything up.

There's no lower rates bro. It's all based on a specific drop rate, which is defined as about 1~5%. That includes any manner of FATE completion -- gold, silver, bronze, success or failure. If you remember, I was just at it non-stop; I stopped doing anything else in game and just hit FATEing hard until it was done. I also didn't subscribe to "I'll do FATEs in area Y this for X number of hours a day," instead I played like "I'm going to FATE in this area nonstop until I don't care about playing anymore today," and completely stop for the day when I obtained an Atma. Usually it lasted 2-6 hours, then I'd give up and stop playing for the day or get an Atma. If I didn't get an Atma, I'd be back in the same zone the next time I felt like playing. Setting arbitrary numbers of farming time will only discourage you, because it will raise your expectations of seeing a drop within that time frame. If you go in with absolutely no expectation of a drop, you'll be able to go at it longer and be pleasantly surprised when it does drop finally.
