    You get the item and use it. It is then permanently on your character. You right click someone on your friends with "New Adventurer" status and you have an option to teleport to the atheryte in the zone they're in.

    Yes it can be used as many times as you wish.

    No there is no cooldown.

    It works with ANYONE on your friends list that is new adventurer.

    To answer someone elses question, yes they must be online.

    Also it doesn't work like Eternity Ring, it doesn't teleport you on top of them. It teleports you to the Aetheryte in that zone, therefore the zone they're in must have an Aetheryte or it won't work.
