what am i doing wrong?

During primal ex fights that involve tank swaps I'm doing something wrong. Picking up initial aggro is fine shield lob into halone combo, then just spam halone combo until tank swap. The other tank grabs the boss at the appropriate time, then when its time for me to tank the boss is where I'll lose it dps will grab aggro off me, healers take aggro of me. So for the transition I will have a charged halone then provoke when its time and use the halone even with fight or flight upthis still happens. I do understand the mechanics of provoke.

just to go back over provoke mechanics as I know them as when you use provoke it puts you at the top of the aggro table +1. That's the simplest explanation I have for provoke. Also to make sure I'll say shield oath is up. For specific scenarios I'll go over what I do for each primal (I don't tank coil cuz of this stuff) for garuda if I'm ot when chirada spawns that's easy to pick up, however when I need to grab suparna + spiny I'll lob spiny and provke lob suparna
This doesn't always work so not sure what I'm doing wrong. During titan I'll voke after landslide and that works well or voke after pushback when its safe again titan isn't a problem cuz when I voke him back everyone is running away or breaking a gaol so dps has pretty much stopped and healers aren't using big heals cuz of low damage. Ifrit is the same as titan not having a problem with this fight as the dos and healers have to move around alot while tanks are swapping. Leviathan I'm picking up/ stun adds again not difficult cuz flash + circle of scorn picks up small adds and stun lock the caster I don't really have to "tank", then the spumes you just flash and pull them away pop a cd and live through the damage. Moggle mog again all tanks are doing in this fight are bouncing the king back and forth while moving away from moogles to avoid buffing them. Which bring me to Ramuh, tank swap is easy orbs drop eat 3 get buff voke halone and then fail. Again when I voke then shield lob or halone(charged or not) dps pulls the boss off me.

So hope some one can help me with this I'm totally lost.
