Easy: Engine that doesnt care about physics, clipping or..logic. lol. Also I had to look for the monster, I only seen various particle effects.
I'm still looking...
On Ragnarok it was a mess, mob resetted 3 times, people dying everywhere cause we couldn't either see him or avoid his attacks. :<
what am I looking at? I can't make it out? All I see is fireworks... (& now i'm singing Katy Perry).
99% of the time that math is nothing but a drain on the servers for no tangible benefit. Developers must make choices when they design games. Sometimes these things must be cut so that servers can actually support hundreds of players instead of making sure something doesn't clip.
Easy fix. Change the spawn location a bit.

Pretty much. :P The thing on the first pic is the NM tail, but other than that... yeah, impossible to see/target.
We were basically 100 ppl in a tiny-ass tunnel trying to target an NM that went through the walls/roof, it was awful even for S rank standards.
He spawned in between the two bridges near Overlook yesterday. Needless to say the concentration of players was so dense in that little platform that he kept popping in and out. Couldn't even see his ability telegraphs, everyone getting hit by his aoes because if you stood back on either of the bridges, he probably wouldn't load at all.
