Steps:
1. Play the game normally on PC with the cross-hotbar with a Xbox 360 controller or a PS4 controller using basic or Scarlet Crush drivers to emulate a 360 gamepad
2. Notice while playing, either in crafting, moving through menus, or in combat that occasionally an input will be held for an inordinate amount of time. This can manifest as your character running as if auto-run were held, you can't cancel out of a menu, or a button appears to be held down.
3. During the time when this bug is manifesting, no other input from the controller will work.

A few issues related to this have shown up in the Technical support forums, but I've seen no bug report yet:

I first noticed this in patch 2.28 and have tried different controllers, different cables, and different ports to try and resolve this problem, it does not happen in any other game
