I was confused about this, did you need to apply somewhere to get the promo code?
Or is it sent to all active players? Or only a few?

It was sent to players who's subscription have been active no later than june 26th. You don't have to sign up or anything just be registered with an active subscription to ffxiv. I've received mine, They are sending them out everyday. I wouldn't panic.
Now is a perfectly good time to be concerned since today and Monday are the only days left where people are in the US offices. I know that of the 9 people I have asked no one has received theirs so idk what's going on.

Not really, they said everyone with an active subscription would get one. Drowning yourself in a glass of water. The day still isn't over yet and you don't know if they have plans to automatically send them out over the weekend via auto mail or something….

All of the emails have been sent out and should have been received by now.
If you're an active North American FFXIV subscriber and did not receive your code, it's possible that it was blocked as spam by your email service provider, or it may have bounced as an inactive email address (We sent it to the email marked as "Email Address 1" on the Mog Station).
However, if you meet the criteria of being an active NA FFXIV account holder, please contact our Support Center and they can assist you! Visit the Support Center here and select "Pre-game Inquiries" for the Contact Category and "Registration Code" for the Contact Sub-category.
