We likely won't know until the patch.
Leviathan EX and Moogles EX are unrelated to the Garuda/Titan/Ifrit EX quests and the latter aren't going to be required for the former anymore. It would be nice if Ramuh was his own thing since he is unrelated to the other two. His drops are i100 so I could see it going either way.
Hard mode will most likely be "do the story". If we want to follow the trend of previous Extreme Primals, Extreme will require that Hard be beaten of course as well as the defeat of either Leviathan and/or King Mog Extreme.
There's no confirmation, but it would be odd if Ramuh Extreme has no gate from the outset. It could be argued that Levi/Mog are "unrelated" to Ramuh, but it can also be argued that Garuda/Titan/Ifrit are unrelated to Levi and Mog and they were a gate anyway. I'd say expect one and then you can be pleasantly surprised if there isn't.
You need Levi and Mog EX clears. It isn't on the English preview page (reqs are listed as ???) but it was on the Japanese one.

Hard mode: finished a new mainstory quest (if i would translate it it's something like "search for the mystery" or "tracking of the secret" or something like that)
EX mode: finished the new mainstory quest "defenders of eorzea", Levi EX quest and Mog EX quest
