Points goes to the person who meets one or more of the following:
1. Saves me when I do something stupid (happens a bit too often, lots of opportunities here!)
2. Are friendly/helpful to the group
3. Are funny/social
4. Are good at their job
If none of the above applies: Random person that was not actively working on being an asshat

........ Well first if the character is male they take priority. Then I pick the best looking one and if he doesn't mess up he, or she cause females playing males is a thing, they get my comm. If they mess up the other male gets the comm. If one of the males has the ugly scrunched up cat face that no one should pick but way too many do and they are the only male option the female character that did the best gets the comm. If there are no males the female that did the best gets my comm. If they all messed up then I give it to a random person. If they all really really messed up then no one gets a comm because I am not rewarding that, this includes males really really messing up no matter how much I like the look of their character.
Edit: while this comes off as unfair, I play dps so no matter how well I play comms tend to go to the tank or healer so I don't feel bad about gameplay not being the deciding factor for comms.

I comm noone cause I'm leveling a monk and all I see is other DPS NOT giving me goad, NOT aoeing, NOT doing the best they can to help me kill shit faster, healers not healing and dying to aoes they don't move from. Yes healers. Tanks either taking too many mobs they can't control making me the monk tank while they do a 1 2 3 combo and dying or doing the babyist of baby pulls making the dungeon take 11 hours 45 mins and 1000 secs and astros giving the dps who's literally doing nothing ALL the balances and thinks giving me the monk an arrow card is helping. NONE of them are getting rewarded from me. Dont do any of that shit and I'd give you a comm easy.
I always commend a random DPS, because commendations are pointless and I like helping people get the achievements quicker.
Unless the group as a whole just really annoys me, I always toss a commend to someone.

I notice people seem to toss commends to healers and tanks just by default - as if the two easiest roles in the game needed a special gold-star just for showing up... (I have mained tanks and healers in MMOs since 2005 - and yeah, there's not a single MMO out there where the hardest tank or healer to play isn't easier than the average DPS, even here where part of the task is to also maximize DPS, the rotations for that are even easier than a redmage).
If I can find a leaf, and they played even up to "OK" they're the likely target of my commendation. Especially if they did something to show a good attitude.
- otherwise it tends to be somewhat random through I tend to disfavor giving it to a redmage or a samurai as those two are super-easy...
I only give it to a tank or healer if they do something unusually good or show a particularly helpful attitude or are the only green-leaf in the group.

Lalafell? Yes
DPSing healer? Yes
Mass pull/speed run tank? Yes
Nice glamour? Yes
I always commend.
If somebody truly goes the extra mile, I will commend them.
If it was a totally normal run, I commend tank/healer if I'm tank/healer, DPS if I"m DPS. If given multiple choices, I commend the individual most like myself (if I'm ranged DPS, will preferentially commend another ranged DPS, for example).
If the person I would normally commend was a total screwup, I pick someone else, often arbitrarily.
