Do something that gives a title (Monster Hunter is actually Notorious Monster Hunter, from killing 25 Notorious Monsters during 1.0)
Open you character page (C on Windows), go to the Profile tab, and click the arrow next to Title to have it list your titles. Selecting one will make it appear with your name.
First off, you have to complete the Achievement that awards the particular title. (You can check the Achievements window to see which you've completed, your progress towards completing any of the others, and what rewards each gives, if anything.)
Then, on the character window, there's a little button at the top next to your character name that lets you choose from among the titles you've earned, selecting which (if any) you want to make your active title.

On a somewhat related note, does anyone know why sometimes titles appear above a player's name and other times below it?

The title will appear on top or under your name depending on the prefix. "Flamelord" would be over your name, but "the Last Resort" will show up under.
It depends on how you'd logically say the title. Typically ones that go below start with "of" or "the", for example Eyriwaen Zirhmusyn the Wanderer's Shadow.

On the topic of achievements, I found my way to the waterfall, and talked to the guy there to get my achievement awards.
However, nothing happened, it just kept repeating the same message telling me to tell him tales. (Or something along those lines, I will screen cap later, I am at work now) I am a level 31 PUG and have completed a TON of achievements, so this seemed strange to me, I expected SOMETHING.
When you get achievement awards, how do you accept them?
The Jonathan at Alapku Falls is the right NPC to talk to however note that not every achievment will give you a title - check in the Achievment list which ones will give you any rewards (titles or items) and try to work towards some of them.
He's the guy you talk to when you complete one of the achievements with an item reward (no need to for title rewards), so you can pick up the item. In my case the first item reward I got was the Master Miner Ring, for gathering 50 different types of items as a Miner, which occurred when I was around level 30 on MIN. Taking that as an example, there were earlier achievements, "I Found That: Miner I" and "I Found That: Miner II" for gathering 15 and 30 different types of items respectively, which did not give any rewards, but it was the final one "I Found That: Miner III" which gave the ring as a reward.
That's a common pattern, where the first few achievements in a given series don't reward anything, but then a later one does. You probably just haven't gotten to the end of an achievement series like that yet. You can check on the Achievements window to see which ones give titles and which give items.
I have some questions about this, we have to talk to that npc first to see if we got the titles? and is there some sort of an achievement tracker, especially for the ones that involve ( for example) completing 1,000 FATES, etc...
Type /achievements to see achievement progress, or use the action(?) in your General section. Titles are unlocked immediately without talking to the NPC.
