I'm just coming back to the game after 6 months away, but don't the fairies auto heal even when they are in Obey stance?

Yeah, but only when people drop below 70% health.
Which means when the fairy is on obey, it's not healing pointlessly or grabbing agro with heals before a fight properly starts.
