Yes, you have a graphical error. FFXIV uses a Moire pattern overlay to emulate transparency. It's probably the ugliest thing I've seen since RAGE's horrifically pixelated textures. As far as a fix for your issue is concerned, it would help if you provide the graphics card model, driver version, OS version, hard drive type, any history of malware or viruses, and let us know when the last time you installed Windows Updates occurred.

As far as the windows update, I update whenever I get the prompt, but I don't remember when last that was. Like the person above me, I do use a laptop, which is about 3-4 years old now, so if I had to guess that's the cause of this. Since I plan to upgrade soon, I'll have to confirm once I have upgraded to see if it persists. Thanks.
