Issue with Achievements - Order of the Twin Adder - The Willow Leaf

Never filled a Bug Report before... hope I did it right...

Anyway, the issue I am having is that I am unable to get the achievement reward for The Willow Leaf under Grand Company > Order of the Twin Adder in Achievements log. Below I have listed steps to the best of my knowledge on what I did, since this happened over 60 days ago due to the 30 day limit on changing Grand Companies, I am sorry for the delay in submitting this bug report. I wanted to try figuring it out on my own first, you know, User Error and all that.

Steps:
1. Doing Supply/Provisioning Missions for the Grand Company - Order of the Twin Adder.
2. Turned in requested item(s) to complete track - The Willow Leaf (200/200).
3. Changed Grand Company to Immortal Flames before talking to Jonathas to get reward, but it was not listed when I talked to the NPC.
4. Figured it was because I changed Grand Company so I waited till the 30 days to change back, but forgot and changed to Maelstrom.
5. Was able to change back to Twin Adder today and still unable to get the achievement reward.
6. Achievement list shows 200/200 but will not grant me reward when I talk to rewards NPC Jonathas in Gridania.
7. Achievement title track starting from - To Each According to His Need: Twin Adder III -to- The Willow Leaf is grey as if not achieved and I no longer have the title "Serpentbringer", But progress bar still shows 200/200.
8. Completed another Supply/Provisioning turn in and still no change.

I can share a screen capture of the issue if requested. Thanks in advance for any and all help!
