An issue wherein players would not receive a penalty for leaving a duty while other players were still loading into the instance.
While this issue was a real problem for people who would drop group the instant they got into a dungeon because they didn't want to run it, I think it may have had an unintended side-effect:

I just queued up for a low-level roulette dungeon, fully intending to finish it for the myth tomes and spare cash, a few minutes ago. All four members load in just fine and the healer starts buffing until he's suddenly 90k'd (or at least it appeared to be a 90k). Unsure if he'd get back, the remaining three of us decided to drop because who knows if he's gonna be coming back. I'm the first one out, and I'm apparently hit with the 30-minute leave penalty.

One or twice in the past, I've been in a dungeon where one person DCs before we're allowed to properly kick them, been the first out and have not been penalized. If this is unintended, I hope it gets resolved soon.
