contact us forms will not load

when trying to use the "report harassment", "report cheating", "report a bug", or "Leave a suggestion" forms from the "Contact us" tab under support, the forms do not load.

form pops up with "title" and "body" but the template does not load and text can not be entered into the form.

I can confirm having had this issue myself. Didn't report because it seems to correct itself from time to time. I suspect it might be tied to Server Traffic, because I once had the unloaded form and it loaded after less than a minute.
