If you can look at your CPU and GPU temps when under load, heat can cause a dip in FPS.
If you're seeing obvious rubber banding then it could very well be internet. For that card 30-40fps is probably pretty reasonable on full settings. Have you tried dropping the detail down and seeing if it makes a difference? For checking your latency I'd suggest getting the trial version of WTFast and watching the ping graph, it will clearly show any problems you have with latency or rubber banding.
Let me list off some basics:
Firstly, check to see if you have the latest drivers for all your components. Make sure your GPU driver is compatible with your Motherboard, and only download the driver from your respective provider. Secondly, check the temperatures of your components when they're in heavy load. There are plenty of free online software that can monitor your fans, your processor, how much RAM is being used, etc. If you're looking for a all-in-one type program, I would recommend CPUID HWmonitor. Lastly, check the settings of your game through your GPU control panel. Nvidia has plenty of ways to customize your in-game settings. Is vsynch on? Do you have anti-aliasing problems? Could you have wacky rendering settings up that are preventing from objects loading properly? The problem could be very major or minor, so I would recommend setting everything to default and tinkering until you see a difference.
Hopefully something above will help you out, otherwise I have no idea what it could be without further information.
Mmmm... just to get these things out of the way, there's some questions that need answering:
- Is the low FPS consistent? As in it doesn't matter what you do, you will always get 30-40 FPS.
- Have you tried adjusting certain settings? Essentially a trial and error thing, particularly with shadows. Even if your hardware blows a games potential settings out of the water, it doesn't mean it'll be 100% compatible to do so, due to drivers not being optimal for the way the game is coded and whatnot.
- Do you experience this slowdown with other games, movies, programs, etc? It may not be the game, but something else that is either running or showing signs of faulty hardware.
- Do you have any hidden programs running in the background?
Your internet connection SHOULDN'T be causing FPS issues. The only way I can see that happening is through a rubberband effect that's causing too many things to happen at once (dropped connection that gets reestablished while you're still logged in, so you see things just teleporting around).
There's nothing wrong on your end. I've been experiencing the same thing since the alpha. There basically isn't anything we can do until either a.) nvidia releases ffxivarr specific drivers or b.) dx11 client.

Have you set all video related programs not to auto start in the task manager?

IDK for a GTX 750Ti on max settings (assuming 1080p) 45FPS sounds about right. Yes it surpass the GTX 660 recommended specs but SE doesn't say GTX 660 or higher guarantees you a 60FPS on max settings thou.
Download the FF14 ARR benchmark program and run it on max setting and see what's your benchmark score. If it comfortably crosses the highest tier but still getting 45FPS it might be due to incompatible driver as other said.

1 issue some people have is not having proper chipset drivers installed. with intel you definitely need these or your performance degrades badly
run dxdiag and make sure directx is seeing everything properly
check your video card drivers you may need to upgrade or downgrade for compatibility(remember that newest isnt always going to fix issues but sometimes will)
1st glace GTX 750Ti
The way Nvidia works normally is
750 = just below 660
660 = just below 570
570 = just below 480
There is a big different between 750, 760, 770, 780, and 790.

So really the 750 TI is below the 660
Commendations.
If I play dps I only give it out to other dps.
If I play tank I only give it out to healers.
If I play healer I only give it out to tank.
Only if they should be getting a commendation.
There are always exceptions to the rules!
