You're wasting 5 seconds of your Unchained bonus because you're using IB while it's up. IB doesn't benefit from Unchained so you shouldn't be using it while Unchained is active. It's also for this reason why you should stagger Unchained and Berserk rather than using them right on top of each other.

This the optimal damage set up. Fracture isn't really worth it since it would be displacing either an IB (which is the same potency but instant and has other benefits) or a BB (which is 20 less potency, but it's up front); remember: we're talking burst damage and DoTs aren't burst (especially when they're over the course of 30 secs). Fracture's displacement is the main reason why I recommend never using it. Unless you've got a metric crapton of spell speed, using it is going to cause you to lose Maim/SE for one or more GCDs, which is a larger DPS loss than what you'd be gaining out of Fracture.

The optimal rotation is:

SE>HS(Unchained)>SS(Berserk)>BB(Brutal Swing)>HS(Internal Release)>Maim>SE>HS>SS>BB>Inner Beast(Infuriate)>Inner Beast

The last Inner Beast is a "possible"; if you've got enough speed/get lucky, you can sneak in the second Inner Beast before you get Pacified (you can actually do it without any speed if you time your Berserk usage *perfectly* so that it's activated at the last possible moment).

With that rotation, everything gets the benefit of both Maim and SE, everything (except the starting SE and HS) ignores the Defiance damage penalty (which actually gives you, effectively, 22.5 seconds of Unchained), everything (except the starting SE, HS, and SS; the starting HS and SS are the 2 lowest damage attacks so this is actually for the best) benefits from Berserk, and IR affects the most powerful attacks it can (the 2 t3 combo attacks with the most Wrath stacks and the Inner Beasts).
