If a tank is losing hate because someone touched a mob he wasn't actively combo-ing on he's doing something wrong. A kill order is entirely unnecessary. It's simply a crutch for a bad tank when they use the excuse "they killed in the wrong order." Sometimes it really is the tanks fault. Sometimes it's the healer's fault. Sometimes it's the DPS fault if they can't adjust their tactics to the group. Instead of pointing fingers and blaming people, maybe everyone needs to take a step back and evaluate if there was nothing they couldn't have done better.
Sure, experienced and well-geared tanks can simply pull the mobs and still keep hate on all of them while the DPS does whatever with them. Less geared or inexperienced tanks, however, generally can't and people should work with them. "Bad" is an extremely poor choice of words here. Are there bad tanks? Yes. Calling a tank bad because they are still in the process of learning or gearing is pretty asinine, though.

The point is to clear the content, so everyone should do everything they can to help toward that goal.

True but there are some that just like something just the right way Louis CK said it best "But that only meets 99% of my criteria!". I feel bad for people that get railed on as a tank and everyone else just sit there and lets it happen especially for honest mistakes that have nothing to do with gear checks or them. Wanderer's palace is one such place as much as I hate that place (blame myth grinding) I know every safe spot, aggro range, and odd quirks of some of the mobs. But it never ceases to amaze me when someone blames the tank for dying on the skeleton/ghost/tonberry pull by the stalker because they decided to be the one guy that has to kill just the tonberry first will full buffs before they reach the tank and catches a knife from the berry and the stalker and goes down like Fraser and insults intow.

    Not always ^^
Did a few "Expert roulette" this weekend, ended up with a BLM, a DRG and a WHM in Brayflox. That was the toughest run I've ever made as a PLD. The BLM did 3 flares in less than 20seconds while the DRG was focusing one single ennemy. The WHM spammed Holy as usual.
I spammed Flash like a crazy, ran out of MP and starting the Halone combo on DRG's target to be sure he won't get hate (but it died before I finished it). A few seconds later... I saw "Holy" and "Flare" on the cast bars and was like "Oh Gods help me now !". Lost 3 targets but luckily they died shortly after.
That was the first time I was scared that I would be flamed by the 3 other members, I lost hate on several ennemies I was ready to be kicked instead they just apologize for the fireworks. Got 3 commendations on that run

A good result for a stressful run but obviously if a DPS wants the hate he will have it.

Very true; that just further proves my point. Losing aggro doesn't necessarily make you a bad tank. There's multiple reasons it could happen and everyone really does have to work together to make the party structure function. If DPS is pulling aggro despite a tank's best efforts, they need to back off a bit rather than simply berating the tank. If the tank obviously doesn't understand the basics of their class, a simple explanation is called for rather than abuse. If you're answered with "Don't tell me how to play my class!!!" that's a completely different story, though.

We did T2 a couple of days ago with a tank from my FC that we're helping out on gearing. Since T2 only needs one tank, I went on my dragoon. At the beginning of every fight with a node, he'd Tomahawk it and then the thing would start yoyoing back and forth between him and the DPS, making it really hard for the melee to actually damage it. After a couple of pulls, we realized it was always going after the same person - the bard that was opening with Raging Strikes before the tank even had aggro secured. Derrrrrrrrrrrrrp.
