    latency

for the last week or so i've been getting some really bad lag, ingame my sending always says anywhere around 200-800 and my receiving is anywhere from 800-3000 no idea what that stuff even means but iam sure high numbers isnt good (would be nice if you just gave us a simple latency setting like every other online game)

every other game i play online works fine except FF14

i did a pathping and got this

Those S/R numbers represent the amount of data you are transmitting, not your latency. Like so many others out there, you seem to be experiencing issues along the Level3 backbone used to get you into Canada. All those other games you play online likely aren't going anywhere near that backbone, and those are responding much better because of that.

Your ISP is responsible for the route you are taking to Ormuco's segments (SE's ISP). It really falls to your ISP to resolve this issue, as they are the ones with an enforceable service agreement with Level3... not Square-Enix. Looks like you are using TWC as well---they may also be able to switch you to either Cogent or TATA instead of Level3, as they have purchased routes from those ISP's as well.

Forward your traces to Tier 3 support (not the Tier1 and 2 guys you talk to at the help desk or come to check your local connection--this is an issue upstream that needs to be addressed by T3 or T4, often referred to as Maintenance and Engineering). If you indeed are subscribed with TWC, this should not be a foreign matter for them. There have been many screaming at them for quite some time about their routing issues--some of it for XIV, others for GW2, WoW, LoL, TwitchTV, JustinTV, Netflix... the list goes on. It's been especially bad in general on both the East and West Coasts, as well as the Midwest for a long time now. In addition to the NSA mess in Chicago that rears it's ugly head in primetime, there have been BIG problems around both Ohio and New York in general, which is affecting a LARGE portion of the US as well as potential players from the UK. By the same token, there have been recurring issues in Southern California and Seattle as well, which can have a big impact on people in the Pacific Region in the same way during prime times. Texas has had it's share of issues as well. It's getting better as more fiber is getting rolled out and routes are getting updated/purchased to take advantage of the new circuits, but it is slow going at best.

For a more visual demonstration, you can see comments and a map for problems at the downdetector.com site:

That image is captured periodically throughout the day, so it may change as the page refreshes.
The normal interactive map is available here:

You can check other services like PSN and XBL as well as some other ISP's at their companies page:
    http://downdetector.com/companies

You can also see lots of issues being discussed at TWC's own forums as well... some go back a really long way. It isn't something new for them:

You can also visualize it through some of the monitoring at Akamai as well. They have a general usage map, as well as one that you can filter by industry (like gaming or retail use) as well as by region to see the impact on trafic flow throughout the day.

It all boils down to congestion problems, and how well our ISP's are managing it. Basically, the problem is regional and you may be impacted or not depending on whether you are getting routed through a problematic region for the particular service you are using. It isn't about your local connection that gets verified when you call in---it is about problems that are cropping up upstream from your local connection, often times in another state or even another country.
